When you come for your appointment, remember to bring the following:
Any insurance information.
Copies of operation records, medical records, x-ray reports and so on from previous doctor visits.
A letter of referral from your GP, if one has not already been received by our practice.
To cancel an appointment:
Please contact the practice during business hours (07 859 0185).
Please cancel at least 1 day ahead so that your appointment time can be allocated to another patient who is seeking treatment.
